The paper recycling idea initially came from a first-year student Erika Muise last January, who wanted to make a difference. "I thought we are an environmental program; we should be doing something for the environment so I asked Edme`, " Muise recalls. Brownlee said yes right away, thus came about the Awareness & Action on Paper Waste Reduction Management project. Every week the group collects paper from 20 different College departments, shreds about 70 to 120 kilograms, and takes it to the land-fill, between classes. "This is action, " enthuses Brownlee. "We are trying to set a good example. Students are very much appreciated for their effort in sustainable management of the campus. We want to change attitudes across the College so people are more aware. When attitudes change, that is when people learn." Amani Abu Abbaud is a new Environmental Technology student at the College and part of the group. She says this is a great project that needs to be adopted by everyone. "We are trying to educate others by spreading our knowledge, " she stated. Tyson Raffray agrees. He is also partaking in the project and expressed his concern when students waste paper in the computer labs, though he added Pay for Print, launched by the College earlier this year is making things much better. "I once saw a student print out 80 pages in error. This is ridiculous; I'm glad Pay for Print is stopping such incidents. This project is making students more aware." Students unanimously agreed that it would be nice to be provided with a truck for their weekly sojourn to the land-fill, as the huge amounts of shredded paper can be easily transported in a larger vehicle. Brownlee says while the project is still in its preliminary stages and the research component cannot be made public yet, it is easy to see that paper is the biggest proportion of the waste generated by Keyano College. "We set-up a systematic overview and then started the action. I know these nine students are seeds we are planting when it comes to action, because they are capable and know everything is possible. Some infestations such as schistosomiasis and lymphatic filariasis are more widespread but others, such as onchocerciasis, equinococcosis, and cysticercosis can cause severe illnesses. Protozoan infections produced by parasites from the Leishmania genus also cause mucocutaneous diseases in many tropical countries. Chagas' disease also shows some dermatological manifestations. Some non-infectious tropical dermatoses are also important such as skin lesions due to lepdopterism and erucism, as well as some photodermatitis produced by tropical plants. Tr., p. 190 ; Later in August, 2001 ; , when the patient returns ten days early seeking refills and Dr. LeuVoy notes that the patient is "acting out more sometimes mean, " Dr. LeuVoy nevertheless makes no change in the therapy nor does he refer the patient to a mental health care provider. Tr., p. 381-82 and Ex. 30, p. 12, Patient #17 ; Dr. LeuVoy thereafter prescribed Zoloct for the patient, even though Dr. Ebel testified that Zolift was not recommended for use in children; and although the patient reported being sexually active with a promiscuous partner, Dr. LeuVoy did not do any STD sexually-transmitted disease ; testing, nor did he make a referral to a clinic for such testing. Patients with progressive symptoms or suicidal ideation in spite of initiating SSRI therapy with close follow-up must discontinue HCV treatment and receive psychiatric care. Patients with well-controlled depression on antidepressant therapy are ready to start HCV therapy. It is helpful to contact patients' mental health providers to alert them about upcoming HCV therapy and ask their assistance in managing the patient. Addressing insomnia proactively with premedication, and asking patients to report any problems between clinic visits, is important. For patients with past episodes of depression or borderline scores on depression screening tools, prophylactic initiation of SSRI therapy is warranted. Citrolpram CelexaTM ; has been shown to be effective for this purpose and is compatible with HAART regimens. Other SSRIs that are useful include paroxetine Paxil ; generally, and fluoxetine Prozac, Sarafem ; and sertraline Zloft ; in patients without insomnia. Buproprion Wellbutrin ; and venlafaxine Effexor ; can cause anxiety and insomnia. Mirtazapine Remeron ; and duloxetine Cymbalta ; should generally be avoided because of limited clinical experience and potential drug interactions with HAART. Gastrointestinal side effects tend to be mild with the exception of ribavirin-associated nausea when it occurs. Diarrhea loose stools ; is generally minimal and self-limiting. C. difficile colitis can occur in patients on HCV therapy, and should be considered in patients with diarrhea that includes peritoneal irritation, fevers, or bloody diarrhea. Diarrhea that occurs after the first month of therapy is likely to be related to enteric pathogens rather than medication intolerance, and should be worked up accordingly. Decreased appetite and weight loss are very common in co-infected patients on HCV therapy. Excessive weight loss due to decreased appetite is rarely a treatment limiting issue. Prior to beginning therapy, patients should be advised that they can expect to lose 5-25 lbs depending on body habitus. During the course of therapy, reassurance that the weight will return once treatment is completed is usually sufficient intervention. Few patients with persistent nausea can make it through a year of HCV therapy. The child was still not gaining sufficient weight according to his pediatrician; the child was being seen almost weekly. The mother then lost her housing with the co-worker and briefly got an apartment through the efforts of Coalition personnel, but she lost the apartment due to non-payment of rent. The Coalition assisted with funds to pay for utilities during this time. Due to the continued allegations of "failure to thrive, " the court removed the child from the mother and placed him in foster care. During the child's time in foster care, the infant gained weight well, slept well, and began to make and repeat sounds. The foster mother reported that the infant was responsive, happy, and very bright. Doctor's care was changed to another physician during this time. Notations from the Guardian Ad Litem GAL ; assigned to the child noted the infant was doing well. Initially, while the child was in foster care, the mother maintained her job, housing, and attended school. The mother's case manager with the Coalition made arrangements for the mother to have biweekly-supervised visits with the infant. It was reported that the mother would not show up for many visits or would call to say that she was coming and then would not. The case manager from the Coalition would call the mother, and the mother would take several days to return the telephone calls. The foster mother stated that the child would scream when he saw his mother and often did not want to be with her. Upon his return to the foster home, he would be agitated and wake in the night, screaming. A maternal aunt has been an active part of the mother's life. She is called "mom" by the niece. The aunt declined having the mother and child live with her although the aunt was in weekly contact with the mother. The aunt expressed concern about the child's care while the infant was in her niece's care. The aunt did have contact with the foster family while the child was in foster care. It was arranged for the mother to go to parenting classes, but she stopped going after two classes. In early 1998, the mother lost her WAGES benefits due to non-compliance ; as well as her Medicaid benefits for herself and her child. Her school attendance became sporadic and she was dropped from that program despite the efforts of Coalition staff. Coalition staff continued to note that the mother had little bonding with her son. Several overnight visits were then arranged for the mother while the child was in foster care. These visits proved difficult for the mother; she had gotten used to not having the child present. She stated to Coalition staff that she would leave him in his crib when she felt she would lose control. All workers involved in the case expressed deep concern about returning the child to live with his mother. Letters were sent to the Judge by the foster parents, the GAL, and Coalition staff expressing concern at the lack of bonding between the mother and child. REACTION TO EXPECTED EXTENSION OF PATRIOT ACT PROVISIONS Groups opposed to two provisions of the USA PATRIOT Act up for review expressed disappointment at a tentative plan to extend both. The proposed extension was written by a conference committee charged with reconciling House and Senate versions of a bill covering the parts of the act that will otherwise expire at the end of the year. Under the plan, the provision that allows the government to issue so-called national security letters without a judge's approval would be made permanent and would allow for criminal prosecutions of individuals who reveal that they have received such a letter. The plan does not make changes to the second section of the act at issue, the library provision, that were included in the Senate bill. Those changes included requiring the government to demonstrate a connection between terrorists and individuals whose records were sought. The Senate bill also called for another review of the library provision in four years; under the proposal, it would not be reviewed for seven years. The plan does include limited concessions. Those who receive national security letters would be allowed to discuss them with their attorneys, and the government would be required to disclose certain details about how the national security letters are used.
