Comparison of the Endeavor Zotarolimus-Eluting CSS to the Cypher stent in ENDEAVOR III provided the first Endeavor clinical data in a US population. Acute success discussed above ; and 30-day MACE rates favored the Endeavor ZotarolimusEluting CSS over the Cypher stent, with a six-fold lower frequency of in-hospital MACE in the Endeavor Zotarolimus-Eluting CSS cohort. At later time points, the two groups demonstrated similar efficacy, with no statistically significant differences for rates of MACE-free, target lesion or vessel revascularization, or target vessel failure during nine month or 12 month follow-up evaluations. The designated primary endpoint of in segment late lumen loss at eight months was significantly higher among patients treated with Endeavor Zotarolimus-Eluting CSS versus the Cypher stent 0.360.46 mm vs. 0.130.33mm, P 0.791 for non-inferiority ; , corresponding to a higher frequency of insegment binary restenosis 12.3% vs. 4.3% ; . It is noteworthy that the in-segment late loss value of the Cypher stent was lower than the observed values in other recent Cypher stent clinical trials. 1, 2, 3A significant objective in designing the ENDEAVOR III clinical trial was to determine if the angiographic and clinical results seen with the Endeavor Zotarolimus-Eluting CSS in this US clinical trial were harmonious with those from previous international clinical trials assessing Endeavor Zotarolimus-Eluting CSS. Late lumen loss values for Endeavor Zotarolimus-Eluting CSS in the ENDEAVOR III trial were virtually identical to those observed in the ENDEAVOR II and ENDEAVOR II-CA trials, indicating that the performance of Endeavor Zotarolimus-Eluting CSS was consistent throughout the clinical development program, and in particular between trials conducted abroad ENDEAVOR II and II-CA ; and in the US ENDEAVOR III.

Asthma is now one of the most common chronic diseases in the United States the sixth-ranking overall and the leading chronic disorder of children ; with an estimated 20 million Americans that are affected or 6.4% of the U.S. population, 4.8 million of whom are children according to the National Institute of Allergy and Infectious Diseases Fact sheet published October 2001. The National Center for Health Statistics Health E-Stats of Asthma Prevalence, Health Care Use and Mortality 2000-2001 indicated that there were 465, 000 hospitalizations, 1.8 million emergency room visits and 4, 487 deaths in the United States. Of those hospitalizations, children ages 0-4 had the highest rate of 67 per 10, 000, with the overall rate for children ages 0-17, 30 per 10, 000. Adults that were eighteen and older had 12 hospitalizations per 10, 000. The asthma hospitalization rate for blacks was 220% higher than for whites, and the female hospitalization rate was 25% higher than males. Children 0-17 made 728, 000 visits to the emergency room, a ratio of 104 per 10, 000. The highest number treated were children ages 0-4 at 180 per 10, 000. Adults 18 years and older had 54 ER visits per 10, 000. ER visits for blacks were 125% higher than for whites, and females 30% higher than for males. Outpatient visits were estimated at 10.4 million, or 379 per 10, 000. Children 0-17 visited the doctor 4.6 million times or 649 per 10, 000. LODOSYN Carbidopa ; Upon first taking Sinemet levodopa carbidopa ; , some patients experience nausea and vomiting. For some, this problem can persist for months and prevent them from taking Sinemet as prescribed. By taking supplemental carbidopa pills, this problem can sometimes be effectively controlled. MOTILIUM Domperidone ; This medication has not been approved by the Food and Drug Administration in the United States. It is, however, available in Canada. Its primary value for the PD patient is in preventing levodopa-associated nausea and vomiting when extra carbidopa is ineffective. Preliminary studies would suggest that this is generally a safe medication and one that is well tolerated by patients. ZOFRAN Ondansetron ; This is a very expensive medication that is primarily used with cancer chemotherapy to prevent nausea. There are several brief reports of its value in relieving confusion, hallucinations and delusions in PD patients. In general, this medication is very well tolerated, with the major side effects being headaches, diarrhea and fatigue. The high cost of this medication, however, is frequently a prohibitive factor for treatment. HYDERGINE combination of ergoloid mesylates ; This agent was introduced many years ago for the treatment of senile dementia. Despite years of study, there is still controversy whether Hydergine improves thinking abilities and or behavior in those with dementia: Some of the agents contained in Hydergine have dopamine agonist properties and have been shown in both human and animal studies to have anti-parkinsonian effects. The doses used in these studies are, however, many times those customary for Hydergine use in medical treatment. Even at high doses, the ergoloid mesylates have not been directly compared to available dopamine agonists, such as pergolide, to see if they have any special advantages. Hydergine and its related compounds are claimed to have anti-oxidant properties, but the clinical significance of these effects is unknown. Side effects of treatment include nausea, stomach upset, headache and low blood pressure. At high dose, these agents may also be associated with spasm of the blood vessels in the arms or legs. BOTOX Botulinum toxin A ; This is one of the most potent biological toxins known to man. When injected into a muscle, botulinum toxin poisons nerve endings, causing muscle weakening and wasting, which can persist for several months. Some patients with Parkinson-related dystonia can have the severity of their dystonia reduced by receiving injections into the dystonic muscles. Treatment with this agent is very expensive. FLORINEF Fludrocortisone acetate ; This is the most commonly used medication to treat symptoms of orthostatic hypotension, which is a severe drop in blood pressure caused by standing. Florinef causes the body to hold onto salt and water and can increase the volume of fluid within the circulatory system. Ankle swelling and weight gain are expected side effects of treatment. Persons receiving Florinef should cautiously increase their salt intake to enhance its actions. Potassium supplements are additionally indicated in many patients to prevent potassium levels from falling dangerously low. By the way, the Chinese character for "breath" has two components, one meaning "exhalation, " the other "inhalation, " and exhalation comes first. 3. Letting yourself be breathed. It is best done while lying on your back, so you might want to try it while falling asleep or on waking. a ; Close your eyes, letting your arm rest alongside your body, and focus attention on the breath without trying to influence it. b ; Now imagine that with each inhalation the universe is blowing breath into you and with each exhalation withdrawing it. Imagine yourself to be the passive recipient of breath. As the universe breathes into you, let yourself feel the breath penetrating to every part of your body, even to your fingers and toes. c ; Try to hold the perception for ten cycles of exhalation and inhalation. Do this once a day. 4. This breathing technique is an ancient one from the yogic tradition. Place your tongue to inner surface of the upper front teeth, then slide it just above your teeth until it rest on the alveolar ridge, the soft issue between the teeth and the roof of the mouth. Keep it there during the whole exercise. Now exhale completely through the mouth, making an audible sound a whoosh ; . Then close your mouth and inhale quietly through your nose to a silent ; count of four. Then hold your breath for a count of seven. Finally, exhale audibly through the mouth to a count of eight. This constitutes one breath cycle. Repeat for a total of four cycles, then breathe normally. If you have difficulty exhaling with your tongue in place, try pursing your lips; you will soon get the knack of how to do it. Note that the speed with which you do the exercise is unimportant. What is important is the ratio of four, seven, eight for inhalation, hold, exhalation. You will be limited by how long you can comfortably hold your breath, so adjust your counting accordingly. As you practice this exercise, you will be able to slow it down, which is desirable. Do it at least twice a day. You can practice this Relaxing Breath anywhere, but it you are seated, try to keep your back straight. I do it the morning, before I meditate, and in the evening, when I lying in bed, just before going to sleep. It helps me fall back to sleep if I happen to wake during the night and it also helps me disengage from thought and calm emotional upset. I think of it as king of tonic, a spiritual rather than a material one, with wonderful effects on the involuntary nervous system. Specifically, it increases the ratio of parasympathetic to sym. Since this phase has normally a lower workload than induction and recovery, the anaesthesiologist usually takes time to complement the record for the previous phase. Today, when vital signs are automatically recorded, the anaesthesiologist must pay attention to the validity of this data, and also needs to enter data regarding the drugs used changes in concentrations, injections, etc ; , and main data regarding the surgical act at least start end of surgery ; . This phase is a lonely one for the anaesthesiologist. In case there was some help during the induction phase, the second anaesthesiologist has left the operation room shortly after the maintenance has started. Moreover, other nurses are usually more concerned by the surgical act than by the anaesthesia. The vigilance and activity level of the anaesthesiologist may vary, during the different phases of the surgical act, but also from one patient to another one. For critical cases i.e. high ASA classes ; , the anaesthesiologist will anticipate more on what could go wrong. In general, one could say that there is a constant need of anticipation; for instance: the inertia of the body's reaction to drugs requires proper temporal model. The anaesthesiologist also needs to follow the surgical act, as this information will be used in order to anticipate the beginning of the recovery phase [de Keyser & Nyssen 1993]. A further 40 per cent of the company's 700m savings target is to come from overhauling its sales and marketing initiatives so that new sales structures and techniques reflect the changes in the company's portfolio, as well as providing a more "tailored and customised" approach for both developed and emerging markets. R&D cuts R&D has escaped relatively lightly, picking up the final 20 per cent of the anticipated savings but only through simplifying and streamlining support infrastructure. Investments will continue to be made to drum the most out of the company's late stage pipeline and the future growth areas of biopharmaceuticals, vaccines and oncology, with the predictable footnote regarding the possibilities of emerging markets in Asia. "In terms of R&D, we have to talk first about where we are going to super invest, " said Garnier. "We are building biologicals, first of all with some in-licensing but also with our own molecules.Of course we will also accelerate our establishment of R&D in China." Only earlier this week, in fact, the company announced a multi-million dollar deal with Tolerx to develop and commercialise otelixizumab, a humanised anti-CD3 monoclonal antibody. Garnier pointed out that the company is looking at up to new launches over the next three years, with products "that will come right on time.to replace what we are losing to the generics." GSK may be luckier than its counterparts in this respect, having a few options that could help buffer the generic onslaught. Others, namely the uncertain leader of the pack, Pfizer, have suffered blow after disappointing blow, leaving their defence seriously compromised. However, GSK has not been without thorns in its side. Avandia, the company's Type II diabetes drug, was the subject of an unflattering appraisal in the New England Journal of Medicine NEJM ; back in May that claimed the drug was linked to an increased risk of heart attack and death from cardiovascular causes. Although GSK emphatically denies the article's allegations, it hasn't prevented Avandia losing over half its prescription market share. Since publication, sales for the quarter are 38 per cent down on this time last year to 225m, with a 48 per cent drop in US markets. In July, a US Food and Drug Administration FDA ; advisory committee met to discuss the potential cardiovascular risks associated with the drug and thiazolidinediones TZDs ; in general. The FDA is currently considering the committee's recommendations. Generics have, of course, played their role in thinning out product revenues as well, with competition for GSK's Zofdan ondansetron ; , Wellbutrin bupropion ; and Coreg carvedilol ; all gnawing away at the branded products' revenues. Zofran was hit particularly hard, with sales down a whopping 86 per cent for the quarter to 32m, along with Wellbutrin dropping 41 per cent and Coreg down 20 per cent compared to Q3 last year. All of these blips contributed to a 6 per cent drop in operating profit over the quarter, hitting 1.91bn. For the year to date, operating profit was down around 2 per cent to a touch over 6bn. Pre tax profit for the quarter was down about 7 per cent to 1.88bn, with year to date figures showing a 3 per cent drop to 5.92bn. With the firm's over-the-counter OTC ; business performing well, these losses came almost exclusively from the challenged pharmaceuticals division. While GSK's Operational Excellence drive is due to bring in pre tax savings of 350m by 2008 and 550m by 2009, the one off charges associated with implementing the programme are expected to be a hefty 1.5bn. The costs will be incurred over the three year period to 2010, with around 70 per cent in cash expenditures and 30 per cent in accounting write-downs. Andrew Witty, the company's recently appointed CEO designate, is due to take over the reins at and dilantin.

In addition, certain benefits administered by the Pharmacy Manager are subject to Prior Authorization. Please see the Prescription Drug benefit description for details. 2. Case management: The case manager will work with You and Your physician to determine the appropriate level of care You need. C. BlueChoice Network The Pool has selected the BlueChoice Network as the Pool's Preferred Provider Organization PPO ; . Although You may choose any medical provider or hospital, You will save money by using providers from the BlueChoice Network. If You choose a BlueChoice provider, the Policy will pay a greater coinsurance rate and the BlueChoice provider's rate will be based on the Allowable Amount for that provider's service. If You choose a medical provider or hospital not participating in the BlueChoice Network, the Policy will pay a lower coinsurance rate for covered expenses and there is no Coinsurance Maximum for covered expenses from Non-Preferred Providers, including ParPlan Providers. Also, Covered Expenses of Non-Preferred Providers, paid by the Policy, will be based on the Allowable Amount, determined by the Administrator, which may be less than the provider's billed rate. The provider may bill You for the difference between the charges paid by the Policy and the provider's billed rate balance billing ; . If this occurs, You will have a greater out of pocket expense. If You choose a ParPlan Provider, the Policy will pay the Non-Preferred Provider level of benefits, but the ParPlan Provider has agreed to: file Your claims; not bill You for the difference between the ParPlan Provider's charge and the Allowable Amount covered under this Policy for any treatment or services; and not bill You for treatment or services that are not Medically Necessary, as determined by the Administrator. There are other advantages to using BlueChoice providers. They will handle the initial paperwork so You do not have to file claims. They may also precertify benefits for You, although it is ultimately Your responsibility to ensure that Your services have been authorized by the Pool. A list of Preferred Providers in Your area is contained in the Preferred Provider Directory that was provided to You. You may call the Administrator's precertification referral department at its toll free number, 888-398-3927, to obtain the name of a Preferred Provider outside Your area, if needed. Any changes to the list of Preferred Providers will be provided to You not less than annually. You may call the Administrator during regular business hours to receive a current list of Preferred Providers. The list of Preferred Providers changes from time to time so it is important for You to verify the network status of Your providers. You can do this by confirming with Your provider that the provider is a member of the network or by calling the Administrator or checking the list of current Preferred Providers found on the Pool's web site txhealthpool.
